Before You Start
- Review your Welcome Letter to confirm your start date, time, location, etc.

- Required Forms - Print and complete all the forms in this section.
Please make sure all documents are signed and dated to ensure completeness. Form Name Description Agreement to Authorize Direct Deposit of Paycheck Include your name, sign and date. Authorization for Direct Deposit of Pay (ADM-4280) Fill out completely and bring a voided check or deposit slip. Employee's Withholding Allowance Certificate (W-4) Include your name, address, Social Security number and number of exemptions. Refer to the online tutorial Payroll Taxes and Federal Income Tax Withholding for additional instructions. Employee's Withholding Exemption Certificate (IT-4) Include your name, address, Social Security number, school district name and number of exemptions. Address Change/Municipal Tax Liability form Include your name, Social Security Number, address of residence and employment. Use The Finder tool, to determine your municipal and school district tax. I-9 Form Federally Required Please check what documents you will need to bring with you on your first day in order to provide proof of eligibility to work in the United States. Statement Concerning Your Employment in a Job Not Covered by Social Security form The SSA-1945 explains the potential effects of two provisions in the Social Security law for workers who also receive a pension based on their work in a job not covered by Social Security. - Review your benefits options.
- You will be eligible for health care benefits and must provide proof of dependency for your dependents within 31 days of your date of hire. Visit DAS Eligibility and Enrollment to identify accepted documents. Once you have created your OAKS password you will be able to upload these documents through the myOhio.gov, Self-Service, myBenefits site, and have access to Job Aids which can help with the uploading process.
