How do I get a password? (For Employers and Representatives)


If you have been using one of the PCs at the IC, you can use the same Employer/Risk number and password (for Employers) or Rep ID and password (for Representatives) to access ICON.

IDs are assigned by the BWC: Representatives receive a Rep ID, while Employers receive a Risk Number. If your organization does not have an ID, you need to contact the BWC to obtain one before you can receive an IC password.

If your organization has a BWC ID but does not have an IC-issued password, you can request one by submitting an Outside Party ICON Access Request Form to the IC. This form is available online (PDF), at any IC office, or you may call the IT Helpdesk at 614-644-6595 (local) or 877-218-4810 (toll free) and request that one be mailed, emailed or faxed to you.

Employer/Risk and Rep IDs and passwords are assigned on an organizational level, not an individual one. Steps in Obtaining an Employer/Risk or Representative Password

  • Complete, print and sign the Outside Party ICON Access Request Form.

    • You must include a SIGNED letterhead along with the form to process your request.

  • Mail or fax the form and signed letterhead to:

  • Once your form has been submitted and the information confirmed, the IC will mail or email your password to you. You will generally receive your information within 5 business days.

    • If you opted to have your password emailed, you will receive a State of Ohio Secure encrypted email. When you click on the link to view your email you will then be prompted to register for a Zix account.
